in the uk old fire places in smaller rooms like a bedroom had these small fireplaces whereas in larger rooms like the drawing room (living room) or kitchen had much larger ones to heat a larger space. they are now worth a lot of money for an original one!

I believe it's a laundry basket as well. Considering the heavy (and not child-friendly) construction, the large industrial caster wheels on the bottom, and it's location in an adult psychiatric hospital... I can't imagine what else it would be.
The crate is a crib. Really - it was on 3 other websites...
