Several ideas occur
1. Wood steps may have rotted out.
2. The contract was written up to remove the stairs by someone who forgot to include the cage or was hoping to get the cage removed gratis. Ironworker's are nice people but they don't do gratis very often.
3. The cage was useful in the removal of the stairs and no one wanted to pay for scaffolding (and the men to build it) to remove it once the stairs were gone.
4. the steps were cement poured on a thin metal form that rusted out as the cement decayed and it collapsed into the pit we can't see.

Rust generally moves very slowly once it has achieved a certain thickness but if there is moving water or something that carries the surface away it will move faster and deeper into the metal seeking to create that skin again.
