These rooms were actually padded cells and could only be opened from the outside.I was at pre nursing college and went on the ward one day and a patient was in one with just a mattress to lie on and nothing else but padded walls

Maybe this isnt an ordinary restroom but a practice one for physical therapy. The shower to help teach patients to wash themselves and the stairs to help patients relearn how to walk up them and build strength.
