I think I remember reading that this was a psychiatric institution for the more affluent members of society (although I don't understand why a private hospital wouldn't be a better choice), so it's possible this was referred to as the Hilton.

I guess in my opinion, I do not see the stain down by the drains where rust could occur, and unles the porcilin table had been cracked or chipped, I would have to guess that the stain is blood and not rust. blood has a dark almost burgandy to black color and it has been put on a light surface, I believe it to be blood placed there, but as motts said sometime after the facility had been closed.
