Amazed, my guess would be that in some instances they did have patients paint some of the murals, as a form of art therapy. Some of the murals were probably done in themes that the staff felt would be conductive to an atmosphere of inspiration for the patients during art therapy or such. They may seem weird to you or me but keep in mind these were in a therapeutic setting and people did the best they could with what they had.
There are members on here who work in the field that can answer this much better than myself but that's just my guess for what it's worth.
I suggest if looking at pictures like that bother you to pass it by and just look at the ones that don't.
