For me, to get in the hospital, you first have to find an abandoned house on the property, go through the window (which has a mattress on the other side incase you fall) go down to the basement (step on a dead cat , no joke), the tunnels are connected from this house to the main hospital. Awesome stuff. And there are more than one tunnel. There are connecting tunnels off the main one.
