My dentist when I was a kid had an xray machine exactly like that, except that the end of it was covered by a plastic cone. I always thought it was a drill and it terrified me - I thought "no wonder people are afraid of the dentist if they put a drill like that into your mouth!" I didn't have a cavity until I was a teenager and found out then that it was an xray machine and not a drill - boy did I feel dumb!
