They were open. Most of these places have open doors (if they don't close automatically); I would guess due to personnel checking that every room has been emptied properly and/or explorers checking out all the rooms.

It's way back in the picture but if you look in there deep enough you'll see a beautiful door with Alice in wonderland like carvings. Very beautiful. Did they usually have chapels in hospitals? It doesn't look very natural...but thats just me. Overall, the picture is very pretty, once again, I love the glass.
