Ok, it took me a few minutes to figure this out; (I'm a little slow sometimes!)
The sign is hanging off the edge of a hole in the floor. I was wondering what that grey board underneath the sign was. The edge of the floor. For some reason, I missed it! Duh....

Wow! The questions this photo brings to mind. Were the blades being used to open envelopes that were sealed tight.... or, was someone writing there own sympathy card as they contemplating taking their own life.....slitting their wrist.... things we will never know.
From the what I read from Motts history of Springfield it was inspiring during the early years that this place worked. I would've showed up for some R&R myself.
