Maybe the door rotted off the hinges and created the hole, that could be why the hole seems to match the doorway.

I love the ceiling. The old fashioned tin tiles look so much better than the tiles they use on new dropped ceilings. Our cafeteria in my old school looked like this but not decayed.

Yes places like this with a lot of rotting wood usually have a musty smell of damp, rotting wood. It usually isn't very powerful except in basements. I don't think it's a bad smell, but perhaps I just have a positive association with it...
