It appears to be an old reservoir for the hospital. Here's a detail of an 1879 map of the drainage and catchment basin of the hospital (South is pointing up) - the placement of the pond matches the orientation of the original hospital building.

I don't think so, this type of door knob would continue on the other side, and the seclusion rooms here had flush doors on the inside.

I'd guess the room was used for storage that was desirable to the clients (personal effects, cigarettes).
